  • Prepare this report using the Quarterly Compilation Report Form.  All items numbered on this annual report form correspond to the numbers on the Quarterly Compilation Report Form.

  • All questions with an asterisk (*) must be answered in order to submit the report.

  • Prepare a separate report for each organized church on the charge.

  • Place a zero in any space for which you have no figure.

  • Use caution in preparing the report.
